Heidi Klum changes name to Heidi Samuel

Model takes husband Seal's last name

Heidi has not indicated whether she will adopt the surname professionally

Heidi Klum is now officially known as Heidi Samuel.

The model has taken her husband Seal's last name.

Heidi, 36, filed the paperwork last month and an LA court approved the request on Friday.

She and Seal, 46, tied the knot in 2005.

They are parents to Lou, five weeks, Henry, 3, and Johan, 2. 

Heidi also has Leni, 5, with ex Flavio Briatore.
