Actress gets help for anxiety and depression

Heather Locklear has checked herself into a rehab facility in Arizona.
‘Heather has been dealing with anxiety and depression,’ her rep tells People.
‘She requested an in-depth evaluation of her medication and entered into a medical facility for proper diagnosis and treatment.
'This is a confidential medical matter and no further statement will be released.’
Last year, Hannah Montana actress Heather, 46, split from her second husband, Bon Jovi star Richie Sambora, 48.
They have a daughter Ava Elizabeth, 10.
She is currently dating actor Jack Wagner, 48.
